A schedule spawns its occurrence at the time of day you set, in the company's own time zone, and the occurrence is due a grace period later. Four hours by default — so a task that appears at 10:00 is genuinely due at 14:00, and everyone knows which it is.
The sweep only touches occurrences still open past their deadline. It marks them overdue and books the miss penalty — three points by default, and zero if you set the penalty to zero.
